Biotechlabore bleiben der Innovationsmotor
Im Biotechsektor konzentrierten sich die Investoren 2021 stark auf die beiden mRNA-Impfstoffhersteller Moderna und Biontech. Dem Nasdaq Biotech Index hat das wenig geholfen, denn trotz der sehr hohen Wachstumsdynamik und der erneut grossen Anzahl von Produktzulassungen blieb er von seiner Jahresperformance deutlich hinter dem Gesamtmarkt. Die Diskrepanz zwischen kommerziellen Erfolgen und unterdurchschnittlicher Performance verleiht dem Biotechsektor eine attraktive Bewertung im Vergleich zum Gesamtmarkt und anderen Teilsektoren der Gesundheitsbranche. Damit stellt sich auch die Frage, welche Kaufargumente jetzt einen Stimmungsumschwung an den Finanzmärkten zugunsten von Biotechinvestments herbeiführen werden.
Ein wesentlicher Grund sind die Patentabläufe, die in den nächsten Jahren insbesondere Big Pharma treffen werden. Wir erwarten deshalb In diesem Jahr einen starken Anstieg der M&A-Aktivitäten, was in Verbindung mit einer sehr attraktiven Bewertung der Biotechwerte und den doppelstelligen Wachstumsraten zu einer Neubewertung des Sektors führen dürfte. Darüber hinaus stammen zahlreiche neue Therapieansätze aus den letzten Jahren aus den Laboren von Biotechfirmen. Die raschen und erfolgreichen Lösungen des Sektors zur Bewältigung der Pandemie wurden mit beeindruckender Geschwindigkeit entwickelt und zeugen von seiner starken Innovationskraft. Diese Lösungen eröffnen auch neue Anwendungsmöglichkeiten. Bei der mRNA-Technologie sind das neue Impfstoffe gegen Infektionskrankheiten und Krebs, die sich bereits in der klinischen Entwicklung befinden und für die in diesem Jahr neue Daten erwartet werden.
Von den etablierten Schwergewichten arbeiten Unternehmen wie Vertex Pharma oder Biogen daran, das Sortiment ihrer bisherigen Wachstumstreiber durch neue Produkte aus ihrer Entwicklungspipeline zu verjüngen. Jede Meldung von bahnbrechenden klinischen Erfolgen seitens dieser Branchenveteranen könnte zu einem positiven Stimmungsumschwung gegenüber dem gesamten Biotechsektor führen. Ein spannendes Feld sind in diesem Jahr die Alzheimer-Therapien, wo klinische und regulatorische Updates erwartet werden. Am wichtigsten werden die endgültigen Daten von Biogen/Eisai für Alzheimer im Frühstadium der Krankheit sein. Der Antikörper richtet sich gegen Ablagerungen des Eiweisses Beta-Amyloid im Gehirn. Eli Lilly wird voraussichtlich Daten aus seiner Phase-III-Studie mit Donanemab präsentieren und Roche und Morphosys könnten in der zweiten Jahreshälfte Phase-III-Daten zu Gantenerumab vorlegen.
In der Onkologie werden in der Klasse der Antikörper-Wirkstoff-Konjugate (ADCs) und bispezifischen Antikörper für 2022 zahlreiche Daten erwartet. Gilead ist bestrebt, den Einsatz seines Krebsmedikaments Trodelvy über den dreifach negativen Brustkrebs (TNBC) hinaus auszuweiten. Hier wird Gilead mit AstraZeneca/Daiichi Sankyo konkurrieren, die im Dezember letzten Jahres vielversprechende Daten zu einem Produkt aus der frühen Pipeline vorstellten. Die beiden untersuchen auch die Behandlung von Patienten mit HER2-armem Brustkrebs mit ihrem bereits zugelassenen ADC Enhertu. Des Weiteren wird Genmab Updates zu seinen Programmen für Lymphome/Blutkrebs geben und Xencor Daten zum Konzeptnachweis für seine bispezifische Technologie vorstellen.
Pharmakonzerne sind eine Bank
Auf den ersten Blick ist die Jahresperformance von 24.2% des NYSE Arca Pharmaceutical Index für 2021 passabel. Massgeblich dafür war allerdings das starke Abschneiden von wenigen Pharmakonzernen wie Pfizer und Eli Lilly. Zieht man Bewertungskennziffern wie das Verhältnis von Unternehmenswert zu EBITDA heran, kommt der Index auf einen um 25% niedrigeren Durchschnittswert als der S&P 500. Zugleich spricht eine Reihe von Faktoren dafür, dass die Pharmakonzerne 2022 mit dem Marktumfeld besser zurechtkommen werden als andere Branchen. Störfeuer von politischer Seite, wie sie gedeckelte Medikamentenpreise hätten auslösen können, sind mit dem vorläufigen Scheitern des «Build Back Better»-Plans von US-Präsident Biden erst einmal vom Tisch. Auch eine anhaltend hohe Inflation kann den Pharmafirmen wenig anhaben, da sie Preiserhöhungen über ihre Produkte weitergeben können.
Zugleich verfügen sie dank der anhaltend hohen Mittelzuflüsse über enorme Cashreserven. Der traditionell hohe Barmittelbestand wird im Fall von Pfizer durch die Einnahmen mit dem COVID-19-Impfstoff und seit Kurzem auch mit der COVID-19-Therapie Paxlovid noch weiter verstärkt. Andere Pharmakonzerne wie Johnson & Johnson, Novartis und GlaxoSmithKline werden durch die geplanten Ausgliederungen einzelner Unternehmensteile einen starken Mittelzufluss erhalten. Diese Milliardenbeträge werden zum einen als Dividende an die Aktionäre ausgeschüttet. Gerade für die Generalisten-Investoren sind die hohen Dividendenausschüttungen ein zentrales Argument für den Kauf von Pharmaaktien. Zum anderen wird Big Pharma kräftig in Kooperationen investieren und die aktuell niedrige Bewertung des Biotechsektors für Akquisitionen nutzen.
Medizintechnik profitiert von Nachholeffekten
Die steigende Lebenserwartung der Erdbevölkerung, der wachsende Wohlstand sowie Innovationen bei Geräten und medizinischen Instrumenten bereiten letztlich den Boden für ein langfristiges strukturelles Wachstum des Medtechsektors mit dem Ziel, die Gesundheitsversorgung besser und effizienter zu gestalten. Wir erwarten Wachstumsraten von 5-6%, die damit deutlich über denen der Weltwirtschaft liegen, zudem sollte die Profitabilität weiter gesteigert werden können.
Die grössten Innovationsschübe im Sinne von marktreifen Produkten und Technologien gab es in den letzten Jahren in der Diabetesbehandlung, dem minimalinvasiven Herzklappenersatz, der Neurostimulation bei Nervenleiden und in der OP-Robotik. Ein gutes Beispiel ist Dexcom. Das auf Diabetesmessgeräte spezialisierte US-Unternehmen ist führend bei den Geräten für die Echtzeitüberwachung des Blutzuckerspiegels anhand der digitalen Datenanalyse. In diesem Jahr steht eine neue Generation dieser Produktgruppe vor der Markteinführung.
Für die Dienstleister und Versicherer ergibt sich vor allem in den USA ein hohes Wachstumspotenzial aus der vertikalen Integration der Anbieter, der wachsenden Zahl krankenversicherter Personen sowie Effizienzsteigerungen infolge niedriger Verwaltungskosten wegen der fortschreitenden Digitalisierung. Ebenso werden die im Zuge der Coronapandemie verschobenen Gerätebestellungen und medizinischen Behandlungen von nichtlebensbedrohlichen Krankheiten wieder Fahrt aufnehmen. Die Summe dieser Faktoren sorgt für stabile Wachstumsraten, welche an den Finanzmärkten die Rolle der Medizintechnik als einer der stabilsten und defensivsten Sektoren festigen.
Digitale Gesundheit wird unterschätzt
Wie in anderen Bereichen von Arbeitswelt und Alltag sind Sensoren, Cloud-Datenspeicher oder künstliche Intelligenz auch in der Gesundheitsversorgung auf dem Vormarsch. Disruptive Technologien verbessern die Behandlung und erhöhen die Effizienz, beispielsweise durch die digitale Analyse von Patientendaten. Im Feld der digitalen Gesundheit geben kleine und mittelgrosse Unternehmen den Ton an. Nach der starken Kursperformance im Jahr zuvor sorgte die 2021 vollzogene Sektorrotation in Large Caps im Jahr 2021 bei den Aktien der Nischenplayer in der digitalen Gesundheit teilweise für Kursverluste.
Die fundamental starke Verfassung im wachstumsstarken Digital-Health-Bereich ist weiter intakt. Bei Umsatz und Ergebnis konnten die meisten Firmen im vergangenen Jahr ihre bisherige Wachstumsdynamik beibehalten. Aus diesem Grund sind die Aktien der Firmen, die in der digitalen Gesundheit tätig sind, inzwischen um 40% niedriger bewertet als noch Anfang 2021. Setzt sich die zuletzt zu beobachtende Rotation zurück in Nebenwerte aus der Gesundheitsbranche fort, steht die digitale Gesundheit vor einem eindrucksvollen Comeback. Hält man sich dazu noch vor Augen, dass die Gesundheitsversorgung weltweit immer noch zu den am wenigsten digitalisierten Branchen zählt, bleiben auch die langfristigen Perspektiven vielversprechend.
In der Summe finden Anleger bei den Gesundheitstiteln je nach Anlagehorizont ein weites Feld an Kaufargumenten. Biotechs und digitale Gesundheit sind tendenziell eher volatile Anlagen mit hohen Wachstumsraten. Pharma dient hingegen als defensive Absicherung in Zeiten steigender Inflation und die Medizintechnik wird von Nachholeffekten für die Zeit nach Corona profitieren.
